Test Name:

MECONIUM DRUG SCREEN PANEL 8

General Information

Lab Order Codes:

MDS (Multiple Drugs)

Synonyms:

MDS Multiple Drugs

CPT Codes:

G0431 – Drug screen, qualitative; multiple drug classes by high complexity test method (e.g., immunoassay, enzyme assay), per patient encounter
Possible reflex:
80102 x each positive – Drug confirmation, each procedure

Test Includes:

Screening for amphetamines, cocaine metabolites, opiates, phencyclidines, THC (marijuana) metabolites, barbiturates, benzodiazepines, and propoxyphene. If a positive is detected, confirmation on the same specimen will be performed at no change. If confirmation is QNS on the original sample, a subsequent charge will be incurred if a new sample is submitted.

Logistics

Test Indications:

N/A

Lab Testing Sections:

Chemistry - Sendouts

Referred to:

MedTox Laboratories (Test# 4043)

Phone Numbers:

Test Availability:

Daily, 24 hours

Turnaround Time:

2 - 4 days

Special Instructions:

If the submitted sample is below the minimum weight (3.0 gm), the ordering provider needs to be notified to collect more sample. Keep pooled samples refrigerated and keep all pooled specimens in one container. If the specimen is short, specify which drugs should be prioritized. Best when performed 2 - 3 days after birth.

Specimen

Specimen Type:

Meconium

Container:

Screw capped, leakproof polypropylene container

Draw Volume:

5 grams (Minimum: 3 grams) meconium

If the specimen is short, specify which drugs should be prioritized.

Processed Volume:

Same as Draw Volume

Note: Submission of the minimum volume results in an elevated reporting limit and inability to repeat analysis.

Collection:

Random sample or pooled random samples for adequate volume

Special Processing:

Lab Staff: Refrigerate specimen immediately after collection and until shipment. Meconium specimens need to be weighed before referring to MedTox.

Do not receive specimens in OER. Give paperwork to sendouts and put specimens in the sendout refrigerator. Sendout staff will receive the specimen when at least 3 grams of meconium have been collected.

Ship refrigerated. Forward promptly. Frozen samples are acceptable.

Patient Preparation:

Best when performed 2 - 3 days after birth

Sample Rejection:

Inadequate specimen volume for testing. Mislabeled or unlabeled specimens.

Interpretive

Reference Range:

Negative for each drug screened

Critical Values:

N/A

Limitations:

● Amphetamines (amphetamine and methamphetamine) screened with the threshold 250 ng/gram

● Barbiturates (amobarbital, butabarbital, butalbital, pentobarbital, phenobarbital, secobarbital) with the threshold of 300 ng/gram

● Benzodiazepines (alprazolam, alpha-hydrozyyalprazolam, diazepam, desmethyldiazepam, oxazepam, ternazepam) with the threshold of 300 ng/gram

● Cannabinoids screened with the threshold 50 ng/gram

● Cocaine (cocaine and benzoylecgonine metabolite) screened with the threshold 60 ng/gram

● Opiates screened with the threshold 150 ng/gram

● Phencyclidine screened with the threshold 35 ng/gram

● Propoxyphene (propoxyphene, norpropoxyphene) with the threshold of 300 ng/gram

Methodology:

Immunoassay (IA)
Gas Chromatography-Mass Spectrophotometry (GC-MS)
Liquid Chromatography/Tandem Mass Spectrometry (LC/MS/MS)
