Test Indications:
Kaletra is a combination of lopinavir and ritonavir. Lopinavir is an antiretroviral protease inhibitor for the treatment of HIV infection. Ritonavir is an inhibitor of cytochrome P450 and therefore slows the metabolism of lopinavir, making it a more effective drug at a given dosage of Kaletra. Because virologic failure of therapy can result from sub therapeutic drug levels, monitoring of antiretroviral drug levels may be important in clinical management of the patient with HIV.

Reference Range:
Kaletra levels peak approximately 4 hours after administration of 400 mg Lopinavir/100 mg Ritonavir. The mean peak concentration is approximately 22% lower for the oral solution than the capsule formulation under fasting conditions. To enhance bioavailability, Kaletra oral suspension should be taken with food.
HIV-1 infected adults, 400/100 mg twice daily x3 weeks.
Lopinavir mean steady state trough:
7.1 + or – 2.9 mcg/mL
Lopinavir mean peak:
9.8 + or – 3.7 mcg/mL
Pediatric Lopinavir levels are similar to those obtained in adults.
