Chemistry

Test Name:

OLIGOSACCHARIDE SCREEN, WBC

General Information

Lab Order Codes:

OLIWB

Synonyms:

Oligosaccharide Screen, Leukocytes

CPT Codes:

82657 – Enzyme activity in blood cells, cultured cells, or tissue, not elsewhere specified; nonradioactive substrate, each specimen

Test Includes:

Screening analysis for several oligosaccharidoses and reported via an interpretive report. Mild sialidosis is not reliably detected. Mucopolipidosis II and III are not distinguishable. Additional biochemical or molecular genetic testing is required to confirm abnormal results.

Logistics

Test Indications:

Investigation of possible oligosaccharidoses

Lab Testing Sections:

Chemistry - Sendouts

Referred to:

Mayo Medical Laboratories (MML Test: OLIWB)

Phone Numbers:

MIN Lab: 612-813-6280

STP Lab: 651-220-6550

Test Availability:

Restricted draw times: Draw Monday – Thursday only and not the day before a holiday. Specimen should be drawn and packaged as close to the shipping time as possible.

Turnaround Time:

8 days

Special Instructions:

Include family history, clinical condition (asymptomatic or acute episode), diet, and drug therapy information. Yellow top tubes are available from the laboratory.

Specimen

Specimen Type:

Whole blood

Container:

Yellow top (ACD solution A) tube
Alternate tube: Yellow top (ACD solution B) tube

Draw Volume:

6 mL (Minimum: 5 mL) blood

Processed Volume:

Same as Draw Volume

Collection:

Routine venipuncture

Special Processing:

Lab Staff: Do Not centrifuge. Blood should remain in original collection container. Do Not transfer specimen to other containers. Specimen should be drawn and packaged as close to the shipping time as possible. Store and ship at refrigerated temperatures. Specimen must arrive within 96 hours of collection to be stabilized.

Patient Preparation:

None

Sample Rejection:

Specimens other than whole blood; gross hemolysis; mislabeled or unlabeled specimens

Interpretive

Reference Range:

An interpretive report will be provided.

Note: This is a screening test; not all oligosaccharidoses are detected. The resulting enzyme activities may be characteristic of a specific disorder; however, abnormal results require confirmation by additional biochemical or molecular genetic analysis.

When abnormal results are detected, a detailed interpretation is given, including an overview of results and significance, a correlation to available clinical information, elements of differential diagnosis, and recommendations for additional confirmatory studies (enzyme assay, molecular genetic analysis).

Critical Values:

N/A

Limitations:

The test can give false-negative results, especially in older patients with mild clinical manifestation. Patients with mild sialidosis are usually not reliably detected.

Additional biochemical or molecular genetic analysis is required to confirm a suspected diagnosis.

Methodology:

Enzyme reaction assay followed by Flow Injection Analysis-Tandem Mass Spectrometry (FIA-MS/MS)
