Patient insurance and demographic data must be included with the request form and specimen sent to the reference laboratory. Also MUST provide clinical history and diagnosis.

Routine venipuncture. Anticoagulant to blood ratio is important. If you use a 10 mL EDTA tube, fill with 10 mL for proper dilution. Under filling of tube will cause cancellation of test.
