Prepare for Your Child's Surgery
What to bring the day of surgery
History and Physical form.

Results of any tests performed prior to the surgery (ex. lab tests, X rays).

Insurance cards

List of your child's current medicines and doses, date and time of last dose, and allergies.

Comfortable clothes for you and your child to wear.

Something familiar and comforting your child wants to bring into surgery, such as a pacifier, blanket, or favorite toy

Special medical equipment, such as a wheelchair, orthopedic braces, trach supplies, hearing aides, glasses, contacts, (remember contact solution, case, and glasses).

A camera (if you like)

This surgery is part of your child's life story. Having pictures of their experience can help them better understand the surgery as they grow up. Also, children love to share their stories and photos. The photos can help them process what they remember and felt about the event. Remember to respect the privacy of others when taking pictures. Please do not include other patient's, families, or staff in pictures without their permission.
