Patient Safety
Your child’s safety is our top priority
Our aim is to provide the best and safest care possible to each child, every time. Our commitment to you and your child is to strive toward that aim. We ask you to partner with us to meet your child’s needs and preferences, and to let us know if this does not occur.
- Partners in care. You are an integral part of patient safety at Children’s. All of our staff looks to families as full partners in providing the best care possible for their children.
- “Nothing about me without me.” We will explain thoroughly the care and medications your child will receive. Your participation in the treatment process is necessary and valued.
- “If it looks wrong, it is wrong.” We will stop and re-evaluate a situation if you – or anyone involved – has a safety concern.
- Disclosure and truth-telling. We will be open and honest, informing you of the potential results of your child’s care, including unexpected results.

Safety is the top priority at all levels of Children’s, including board, management, physicians, and staff.
We encourage everyone – patients, family members, staff, visitors, and volunteers – to alert us to safety concerns. You can voice your concern to anyone on your child’s health care team, or complete a Safety Learning Report, found in cheese-shaped holders on the walls of most units. If you can’t find a report form, ask your nurse for one.
Children’s has an Office of Patient Safety in place to evaluate any safety concern brought up by staff, patients, or family members. The Office of Patient Safety reviews each issue and develops steps to make Children’s an even safer place for your child to receive care. Safety Action Teams are in place on all units at Children’s. These are multi-disciplinary teams of direct care staff: nursing, pharmacy, health unit coordinators, and others. Teams meet monthly to review patient safety learning reports and problem-solve risks and hazards that may emerge.
All physicians and nurses at our hospital are licensed and participate in continuing education.
Orientation for all new employees includes patient safety. A further 90 minutes of orientation on patient safety is required for all new patient care employees.
For medication safety, we double-check to make sure your child receives the right medication, right dose, with the right method at the right time. We encourage you to speak up if you have any questions about your child’s medication.
