How to Refer a Patient

Prior to seeing a patient in the clinic Children’s requests that a referral letter be sent by the referring physician outlining a brief medical history, current issues, and how we might be of help.

Although all official referrals require a Physician or Nurse practitioner order, anyone is free to call us to discuss if our program can meet the needs of a child and family. If you are unsure which program would best suit your needs please call (612) 813-6450 and we will assist you in the process.
