Community Connect Receives MDH Healthcare Innovation Award

Care teams at Children’s Minnesota’s Minneapolis and St. Paul general pediatrics clinics received an award for innovative work in connecting pediatric patients and their families to community resources that support health and well-being through the Community Connect program. In 2017, Children’s began screening patients for social needs as well as their physical needs at well child visits, and put in place a team of resource navigators to help families connect to community resources to address social needs such as housing, transportation, food insecurity, employment, and others.

Children’s Minnesota was one of two organizations honored by the Minnesota Department of Health’s Health Care Homes program at its annual conference on April 10.  A health care home is a primary care clinic or clinician certified by the Minnesota Department of health to coordinate care among the primary care team, specialists and community services to ensure that patients receive what they need to stay healthy and manage their health.
