Nursing: Nurse to Initiate Phased Admission PowerPlan Orders

A new initiative allows Hospitalists (floor or ED-IP) to place phased admission powerplan orders for a patient prior to their transfer from the ED. If the Hospitalists have placed orders in advance of the patient transfer from ED, ED providers will only sign Phase I of the ED Transition of Care orders. Phase 1 contains a conditional order, Conditional Initiation of Admission Orders. This conditional order will give the inpatient RNs in the MedSurg, Limited Stay, and ICC units permission to initiate the Phase 2 admission powerplan orders once they receive the patient.

If the Hospitalists have not placed admission powerplan orders when the patient is ready to leave the ED service, the ED provider will initiate Phase II of the ED Transition of Care PowerPlan. The inpatient RN will notify the Hospitalist that admission orders are needed as was standard practice prior to this change.

The Conditional Initiation of Admission Orders will instruct RN to initiate Phase 2 orders.
