Professional Staff: Nurse to Initiate Phased Admission PowerPlan Orders

A new initiative allows Hospitalists (floor or ED-IP) to place phased admission powerplan orders for a patient prior to their transfer from the ED. If the Hospitalists have placed orders in advance of the patient transfer from ED, ED providers will only initiate Phase I of the ED Transition of Care orders.

Inpatient RNs in the MedSurg, Limited Stay, and ICC units can initiate pre-planned admission orders (Phase 2) and provide continuity of care for the patient.

If the Hospitalists have not placed admission powerplan orders when the patient is ready to leave the ED service, the ED provider will initiate Phase II of the ED Transition of Care PowerPlan as was standard practice prior to this change.
