Updates to the Humpty Dumpty Falls Assessment Scale (HDFS)

The Humpty Dumpty Falls Assessment Scale (HDFS) has been updated. Questions have been added or modified to increase accuracy.

The following question has been added to the Humpty Dumpty Falls Assessment Scale (HDFS):

  • Admit R/T Fall or Hx of Fall Since Admit: Is the admission related to a fall or does the patient have a history of a fall since admission?

The following questions have been updated:

Diagnosis

  • Combined same scoring Diagnosis factors: Psychiatric disorder and Behavioral disorder now Psych/Behavioral disorders
  • Added examples to Alt Oxygenation: Respiratory diagnosis, dehydration, anemia, and dizziness

Environmental Factor

  • Combined same scoring environmental factors to the following options:
    • History of falls or infant/toddler placed in bed
    • Use assist dev, infant/toddler in crib, furniture/lighting
    • Patient placed in bed
    • Outpatient area

Note: If answered Yes to Admit R/T Fall or Hx of Fall Since Admit question, select History of falls or infant/toddler placed in bed.

Medication Usage

  • Combined same scoring Medication Usage factors to the following options:
    • Multi sedati/barbit/phenothiaz/antidep/lax/diruretic/narc
      • Medication examples that increase the risk of falls added: Multiple use of sedatives, barbiturates, phenothiazines, hypnotics, antidepressants, laxatives, diuretics, narcotics
    • One of the meds listed above
    • Other medications/none
