Clinical Highlights will display a new Home Care Patient link when services are received specifically from Children’s Home Care.
This addition to Clinical Highlights will provide continuity of care for the patient and place Children’s Minnesota in compliance with The Joint Commission.
Note: The notification will no longer display 60 days after a patient’s discharge from Home Care Services.
Clicking on the Home Care Patient link in Clinical Highlights will open the most recent Home Care Visit Report. This report includes pertinent information (i.e. patient’s Medication by History list) updated during the most recent visit with Home Care.
The Home Care Visit Report can also be found in the Notes section in the patient’s chart. This report is located within Clinical Documents in the Home Care folder.
Note: To view information regarding home care services received outside of Children’s, click the Case Management Discharge component in the Workflow Summary.
