IView Changes

Several IView changes are being implemented regarding Wound Ostomy consults, the Power Port field in a Dynamic Grouper, new chest tube documentation options, and new assumed pain reassessments documentation options.

Adjustments to Automatically Generated Consults to Wound Ostomy

In an effort to reduce the amount of automatic requests being generated for a Wound Ostomy consult, several IView changes are being made. 

Note: If a Wound Ostomy consult is needed, nursing must place an order for the Screening Request to Wound Ostomy Team.

  • Possible Pressure Injury: The option to select Yes notifies WOC Team of possible pressure injury is being removed from tubes/drains, non-surgical wounds, non-surgical skin conditions, and non-invasive devices.
  •  Skin at Risk of Breakdown Discern Alert: The discern alert that is generated when the SIRA score >=14 is being removed.
  • Admission Assessment-Neonatal: The age parameters for the Gestational Age at Birth field are being modified. A Wound Ostomy consult will now automatically generate at 20-24 weeks. Previously, the Wound Ostomy consult was automatically generated at 20-27 weeks.

Power Port Type Field

The Port Type field within the Dynamic Grouper for a Port will now be a required field.

Prior to creating the Dynamic Grouper for the port, locate the port type information, i.e. Power Port or Standard Port. This will ensure the information will be accurate. If you are unable to locate the port type information, the Dynamic Grouper can be created using the Power Port type of Unknown.

Note: If you selected Unknown and now know the port type, follow these steps to correct your documentation:

  1. Inactivate the incorrect label.
  2. Add new Dynamic Grouper.
  3. Create new label.
  4. Add a free-text comment identifying the information being corrected. For example, Corrected to Standard Port.
    Note: Every Dynamic Grouper has at least one field with a free-text option. Any free text field can be used to document comments.
  5. Re-document the original Insertion Date, last Cap change date, and last Dressing change date. This will ensure correct dates display in the Clinical Summary for line days, Cap change and Dressing change.

IView Chest Band Update

Documentation choices were updated to reflect options in the chest tube drainage system.

Reassessment for Assumed Pain or Assumed Agitation

Implemented November 21, 2023

New options were added to the drop-down for Assumed/suspected pain assessments. Additional choices were added to facilitate documentation of the reassessment response following an intervention.
