Payment and insurance
We will submit all claims directly to your insurance carrier. Your insurance may require a co-pay which we will collect at the time of the visit. We will bill you directly for any portion of your bill not covered by your insurance company.
If you have any questions regarding your insurance or your bill, our business office is available Monday through Friday from 8 a.m. to 5 p.m. and can be reached at 952-992-5560. We have contracts with most major HMOs and PPOs.
Referrals
Our office provides referrals to specialists and will complete the necessary paperwork should your insurance require it. Check your insurance handbook for the approved procedure for referrals. It is possible that a specialist or hospital might be in your provider book and yet we cannot refer to them because of contractual agreements. Most plans control/restrict where you can go, especially for chemical dependency and behavioral health issues.
