Whether your child has been referred to rehabilitation by your pediatrician or because of a hospital stay, you’ll need to pre-register before your first visit. Call your rehabilitation clinic and have the following information ready:
- The reason for appointment
- Your primary physician’s name and clinic
- Your insurance information
- Your availability for an appointment
This call will take 5-10 minutes. After the call, we’ll work with your physician to get a medical order for the services requested.
If you received a questionnaire in the mail, please complete it in advance. Don’t forget to mail it back or bring it with you. You’ll also need to bring any pertinent medical records to your appointment.
