Pre-registration – Surgery, X-rays and special diagnostics
For surgery, X-rays and special diagnostics appointments (you only need to pre-register for special diagnostics if your appointment is at our St. Paul location), you need to submit your pre-registration by 3 p.m. the day before your service.
To pre-register, complete the pre-registration form or call the admitting department. If you pre-register online, you will only be contacted if there is any additional information needed to complete your pre-registration.
