Epic-to-Epic referrals
Place the referral order directly from the referring clinic’s Epic EHR.
EpicCare Link
Clinics enrolled in Children’s Minnesota EpicCare Link can access referral resources and submit information through EpicCare Link. Login to EpicCare Link. Not a user yet, request access.
Fax
Referrals may still be faxed to ambulatory clinics that accept referrals.

Order testing and procedures
Schedule a surgical procedure
If you do not have access to EpicCare Link, please use the surgery request form.
- A history and physical examination must be completed no more than 30 days prior to surgery or procedures. A history and physical examination form is available for your convenience.
- For additional support with highly specialized patients or complex anesthesia consultation, patients can be referred to Children’s Minnesota Pre-Surgical Clinic prior to surgery.

Laboratory: Ordering information
If you do not have access to EpicCare Link, use one the lab requisition forms below and fax the order to any lab location.
- General Outpatient Lab Requisition: Most common lab tests.
- Allergy requisition
- Add a lab test to an existing sample.
Outpatient lab collections are walk-in only with a valid lab order and ordering provider information.
Radiology (Medical Imaging)
Order imaging
Submitting imaging orders through EpicCare Link sends orders directly to the scheduling queue and includes the information needed to schedule appointments, including coordination with sedation.
If you do not have EpicCare Link, use the medical imaging form, and fax it to 612-813-7669.
