Admissions to Children’s Minnesota

There are several ways to admit patients to Children’s Minnesota.

An emergency physician will provide triage for emergencies and trauma. The clinical review coordinator will assist with necessary certification.

A pediatric hospitalist will provide triage for same-day and/or direct admissions. Children’s Minnesota Physician Access will connect you to a clinical team to determine stable condition and bed availability.

To request a direct admission to the pediatric hospitalist team, Children’s Minnesota Physician Access will connect with the pediatric hospitalist to determine stable condition and bed availability. Many insurance plans require prior authorization for a hospital admission. Families are responsible to obtain the required pre-authorization.

To admit a patient, please prepare the following:

  • Patient name
  • Date of birth
  • Gender
  • Address (to assist with identification)
  • Primary care clinician’s name
  • Diagnosis / reason for referral
  • Isolation requirements
  • Special needs requirements
  • Anticipated time of arrival

If you’ve already scheduled an admission, in addition to the information listed above, please be prepared to include the following patient information:

  • Address
  • Telephone number
  • Insurance coverage. Physician’s offices will be responsible for meeting insurance company pre-certification requirements prior to admission.

Additionally, a history and physical examination form must be completed no more than 30 days prior to admission for surgery or procedures and not more than 24 hours post admission.

A clinic appointment sheet is available to direct patients when visiting a Children’s Minnesota clinic. Please give them this clinic appointment sheet to present to the clinic when they arrive.
