Your medical records

Patients, parents and legal guardians have legal right to information about a patient’s care. At Children’s Minnesota, we want to make that process clear and easy for you.

Release of information form

We take every precaution to protect the rights and privacy of our patients. To ensure the privacy of patients and their families and to comply with federal regulations, parents or guardians must sign a release of information form (PDF) in order to get a copy of your medical records or your child’s medical records.

Request a correction or amendment to a medical record

It is Children’s Minnesota policy to permit individuals or an individual’s legal guardian to request that Children’s Minnesota amend protected health information (PHI) or a record about the individual in Children’s Minnesota designated record set for as long as the PHI is maintained in Children’s Minnesota designated record set. Please complete this form (PDF) with a written explanation to support the requested correction or amendment.
