Release of information form
We take every precaution to protect the rights and privacy of our patients. To ensure the privacy of patients and their families and to comply with federal regulations, parents or guardians must sign a release of information form (PDF) in order to get a copy of your or your child’s medical records.
Need an interpreter?
Children’s has interpreters in a wide variety of languages from Spanish to Somali. You can request an interpreter to help you communicate with staff about your medical records.
