The search for nearby treatment facilities is based upon Google search parameters and will return results based upon how facilities manage their website information. If you are aware of closer or more appropriate facilities in which to seek care, use your best judgment in selecting a site of care.

Title to the Content remains with Children's Hospital, Self Care Decisions LLC or its licensors, as agreed between them. If you violate any of these Terms and Conditions, your permission to use the Content and Application automatically terminates and you must immediately stop all use of this Application.

In no event shall Children's Hospital, Self Care Decisions LLC, and each of their affiliates, licensors, suppliers, or any third parties be liable for any damages (including, without limitation, incidental and consequential damages, personal injury/wrongful death, lost profits, or damages resulting from lost data or business interruption) resulting from the use of or inability to use the Application or the Content, whether based on warranty, contract, tort, or any other legal theory, and whether or not Children's Hospital, Self Care Decisions LLC, and each of their affiliates, licensors, suppliers, or any third parties are advised of the possibility of such damages. You (and not Children's Hospital or Self Care Decisions LLC) are solely responsible for any outcomes arising directly or indirectly out of any information that you intentionally or unintentionally share with this Application.

Indemnity. You agree to defend, indemnify, and hold Children's Hospital, Self Care Decisions LLC, and each of their affiliates, officers, directors, employees, agents, licensors, authors, reviewers, and suppliers, harmless from and against any claims, actions or demands, liabilities and settlements including without limitation, reasonable legal and accounting fees, resulting from, or alleged to result from, your violation of these Terms and Conditions or your use or misuse of the Application or Content.

Entire Agreement. These Terms and Conditions constitute the entire agreement between you, Children's Hospital and Self Care Decisions LLC with respect to the use of this Application and Content.
