Collective for Community Health Innovation Fund

80% of what impacts the health of children happens outside the clinic walls. That is why Children’s Minnesota created the Collective for Community Health which aims to improve the health of children and families – both inside and outside of our hospitals and clinics. We know we cannot do this work alone and we are dedicated to working with community organizations to make a meaningful impact in partnership with the communities we serve. 

The Community Health Innovation Fund, managed by the Collective for Community Health, gives Children’s Minnesota an opportunity to invest in organizations that are working to advance health equity and other health priorities identified in Children’s Minnesota’s most recent Community Health Needs Assessment 

Grant criteria

Select non-profit organizations that meet the following criteria may be invited in 2024 to apply for a one-year Community Health Innovation Fund grant of up to $75,000: 

  • Engage community members in creating and/or providing services  
  • Serve communities with high rates of health disparities 
  • Focused on racial equity and health equity; anti-racist 
  • Provide culturally specific care or services  
  • Diversity in leadership and staff, including reflecting the community served 
  • Alignment with Children’s Minnesota’s 2022 Community Health Needs Assessment priorities 

Awardee selection process

Grant applications will be reviewed, and grantees selected by the Children’s Minnesota Community Advisory Council, a group made up of external partners and several Children’s Minnesota leaders focused on informing Children’s Minnesota’s community health efforts.   

Planned Timeline

May 2024
Select nonprofit organizations will be invited submit a letter of intent (LOI). 

June 2024
Letters of intent will be reviewed, and organizations will be invited to complete the full grant application 

July 2024
Applications due 

October 2024
Awardees will be notified 

November 2024
Funds will be released
