Lab/other tests: Hemoglobin; Lead test (as needed); Tuberculosis Test (as needed); Blood tests*
Screenings:  Developmental/behavioral screening


15 Months

Immunizations: Haemophilus Influenzae Type B (HIB); Pneumococcal Conjugate Virus (PCV 13); Diphtheria, Tetanus, Pertussis (DTaP)**; Influenza
Oral health: Oral health check; Fluoride varnish
Screenings: Developmental/behavioral screening


18 Months

Examinations: Physical examination; Weight and length; Head circumference; Developmental/behavioral assessment.

4 Years

Immunizations: InfluenzaPre-kindergarten immunizations may be completed between ages 4-6.
