2 month well child checkup

Examinations: Physical examination; Weight, length and head circumference
Immunizations: Rotavirus (oral); Haemophilus Influenzae Type B (HIB); Pneumococcal Conjugate Vaccine (PCV13); Diphtheria, Tetanus, Pertussis (DTaP); Inactivated Polio Virus (IPV); Hepatitis B (HBV)
Screenings:  Developmental/behavioral screening, Maternal depression screening


4 month well child checkup

Examinations: Physical examination; Weight, length and head circumference
Immunizations:  Rotavirus (oral); Haemophilus Influenzae Type B (HIB); Pneumococcal Conjugate Vaccine (PCV13); Diphtheria, Tetanus, Pertussis (DTaP); Inactivated Polio Virus (IPV); Hepatitis B (HBV)
Screenings:  Developmental/behavioral screening, Maternal depression screening


6 month well child checkup

Influenza – once a year during flu season; twice the first year given
